U.S. Passport Checklist
- Complete and print form DS-11 or DS-82. DO NOT SIGN THE FORMS!
- DS -11 is for first time applicants, all applicants under 16 years old, and for replacement of a lost or stolen passport
- DS -82 is for adults renewing their passport
- Schedule an appointment online and bring the completed DS-11 or DS-82 along with the other REQUIRED DOCUMENTS to the interview.
- REQUIRED DOCUMENTS:
- One 5×5 cm photo taken in the last 6 months against a white background.
- The photo cannot have been used for a previous passport.
- No eyeglasses or head coverings.
- One 5×5 cm photo taken in the last 6 months against a white background.
- If renewing a passport, your previous U.S. passport including a photocopy of the front page and any endorsements.
- For first time applicants and children under 16, proof of citizenship:
- U.S. birth certificate, OR
- Consular Report of Birth Abroad, OR
- Naturalization Certificate
- Lost or Stolen Passports. If replacing a passport that was lost or stolen, complete form DS-64 as well as DS-11 and bring a police report translated into English.
- Name Changes. If updating your passport because of a name change, bring a marriage certificate, divorce decree or court-order demonstrating the name change.
- For Children under 16, both parents must come to the interview with the child.
- If one parent cannot appear for the interview, the missing parent must complete form DS 3053 allowing the other parent to apply for the passport alone.
- If one parent is deceased, the applying parent must provide a death certificate.
- If only one parent is listed on the birth certificate, that parent must appear in person.
- If one parent or a third party has been granted sole custody of the child, we must see a court-approved custody order or judicial declaration of incompetence.

- First Time Adult Passport / Renewal: $135.00
- Adult Passport Renewal: $110.00
- Lost or Stolen Passport Replacement: $135.00
- Minor Passport Book: $105.00
